Notice to Bidders

Sealed proposals will be received by the Waverly Health Center (the “Hospital”) and/or Construction Manager – Graham Construction at Waverly Health Center (the “Hospital”) 312 9th St. SW Waverly, IA 50677, Attention: Heidi Solheim on or before 14th day of November, 2019 at 2:00 PM and opened at 2:05 PM the same day in the Waverly Health Center Tendrils Rooftop Garden for furnishing all labor, materials, and equipment for Bid Packages 01 – 11.  The proposals will be presented to and considered by the Board of Directors of the Hospital during a board meeting on November 25, 2019, at 6:00 PM in the Tendrils Rooftop Garden at the Waverly Health Center.  Such proposals received will be acted upon at such time and place or at such later time and place as may then be fixed.

Pre-Bid meeting will be held on Friday, October 11th at 1:00 PM in the Tendrils Rooftop Garden.  (Please use the entrance next to the Red Entrance and take the elevator to the 4th floor.)

PROJECT/WORK DESCRIPTION:  Furnish all labor, material and equipment required to accomplish the construction described in the specifications, indicated on the plans, and per the schedule. The Work includes a new addition to the Hospital featuring Emergency Department, Christophel Clinic, and Outpatient Medical Services.  It also consists of an interior renovation to the General Surgery Clinic, Ortho Clinic, Registration, R/T & CVS, and Administration areas.

BID SECURITY: All bids must be accompanied by bid security payable to the Hospital in an amount equal to at least 5% of the highest total amount including Add Alternates.  The bid security may be in the form of a Certified Check, Cashier’s Check or a bid bond executed by corporations authorized to contract as surety in the State of Iowa.  The bid bond shall be executed by the Bidder on AIA Bond Document or Iowa Guide 31.  Bid Security shall be in a separate envelope with the sealed bid and shall be forfeited to the Hospital as liquidated damages in the event the bidder fails to enter a contract and furnish a bond within ten days after his bid has been accepted.  No Bidder may withdraw his bid for at least sixty (60) days after the scheduled closing time for the receipt of bids.

The successful Bidder will be required to furnish and pay for satisfactory Performance Bond and Labor and Material Payment Bond in the amount of 100% of the Contract Price.  The Bond must be issued by a responsible surety approved by the Board of Directors of the Hospital.

The Notice to Proceed will be issued following acceptance of the executed Contract by the successful Bidder and the requisite Performance Bond and Labor and Material Payment Bond and insurance certificates, and a determination by the Hospital that it has secured permanent financing for the Project and approval by USDA-RD to issue the Notice to Proceed.  All work for this package shall be completed within the given schedule set forth in the bidding documents.

To the extent allowed by Federal law and regulation, by virtue of statutory authority, a preference will be given to products and provisions grown and coal produced within the state of Iowa.  Pursuant to 2 C.F.R. § 200.319, as adopted by the United States Department of Agriculture in 2 C.F.R. § 400.1, no such preference will be given except where applicable Federal statutes expressly mandate or encourage such preference.
