Access Your Personal Health Information Online!

We are now offering our clinic patients easy and private access to their medical information online. You can view your personal health information whenever and wherever you have access to the Internet!

A staff member will give you your username and a temporary password. This will give you access to your personal health record and other helpful features from any computer or smartphone with an Internet connection! Your temporary password is good for 48 hours. After that time, you will need to have it reset by calling one of the clinics.

Call any Waverly Health Center clinic and the receptionist will be able to help you register or reset your password.

Information from the clinic patient portal is only from visits to Waverly Health Center clinics. Hospital and non-WHC clinic information will not be available.
